I have kind of the same problem (video and sound only one way), I have the following session id's for you:

At least one of these gave the problem.

Some additional info, I got these errors:

NetworkError: 502 Bad Gateway - http://anvil-staging.tokbox.com/crossdomain.xml
[ERROR] opentok: TB.exception :: title: Security Error (1009) msg: SessionInfoAPI :: Failed to get session info - Error #2170
[ERROR] opentok: TB._exceptionHandler called but could not find a partner ID.

I hope you can help me out here.

I'm quite sure none of our users has Google Hangouts installed, or even uses Google+, but I'll check.

No Google Hangouts here. We did some more tests. The error is occuring not too often, typically we had five or six working chats before the black box appeared. It did appear on the other side, that is: I suppose PC A has the problem (cause only in chats involving this PC we see it). When B chats with A he can't see and hear A, but A can see and here B.
We did some tests were A hadn't had any web cam plugged in, same error happens: sometimes B can't hear A. So the web cam itself, driver or something, shouldn't be the problem.

When you next see this can you check if your tester is using Chrome 21 on Windows? We have seen sporadic no video in this scenario - one of many problems we're seeing with Chrome 21 and Flash.

no, as I wrote the fault happens while using Firefox.
